2. BLAME VS SYSTEM
   - Am I blaming individuals instead of systems?
   - What process failures enabled the human error?
   - What guardrails were missing?

3. ACTION ITEM QUALITY
   - Are my action items specific and measurable?
   - Will they actually prevent recurrence?
   - Am I proposing band-aids or structural fixes?

4. MISSING PERSPECTIVES
   - What would other teams identify that I missed?
   - What customer impact did I understate?
   - What contributing factors did I dismiss?

5. REVISED POSTMORTEM
   Rewrite the root cause section and action items to:
   - Address true systemic causes
   - Include measurable prevention
   - Avoid blame while ensuring accountability
